HOURS: Wednesdays, by appointment only.

DIRECTIONS: Our Westside Office is located at the above address in Suite 110 at Greenburg Plaza, in Tigard, Oregon. Our office is the first door after you turn left, facing Greenburg Road and is labeled “110”. 

PARKING: We suggest parking in the back parking lot behind the building. The entrance to the parking lot is located on the left side of Greenburg Plaza when you are facing the building. This location has ADA parking in the parking lot and is fully ADA accessible.

TELEMEDICINE

Telemedicine hours vary, but are typically Monday and Tuesday 10:00am to 6:00pm, and Wednesday and Thursday mornings. All visits are by appointment only. To schedule, click the button below.

EASTSIDE LOCATION

4037 NE TILLAMOOK ST.

PORTLAND, OR

HOURS: Tuesdays, by appointment only.

DIRECTIONS: Our Eastside office is located at the above address on the corner of NE Tillamook Street and 41st Avenue in Portland, Oregon. Our office is the door on the corner, across the street from the Library.

PARKING: This location has street parking only. Please note that while this office meets ADA standards, it has no official ADA parking. Please plan to arrive 5-10 minutes before your appointment starts to account for parking time.
