ESTABLISHED PATIENTS
Telemedicine Appointments: To schedule a telemedicine follow-up, click the button to the right. These appointments are for established patients only. If you have not been seen in over 3 years, you are considered a new patient. Please see below for new patient information.
In-Person Appointments: To schedule an in-person appointment or exam, please send us a message through your secure Elation Passport account.
Urgent Appointments: If you need an urgent appointment with Dr. Juniper, please call or text (971) 328-0071 so we can assist you.
NEW PATIENTS
Our New Patient Waitlist is currently full.
If you are on CareOregon and are assigned to Dr. Juniper for primary care, but are not yet established with Dr. Juniper, please contact us immediately.
Otherwise, if you are interested in becoming a new patient, our waitlist will likely re-open in about three months. Please contact us then to inquire. (Any attempts to contact or be added to the waitlist before then will be ignored).
We will not respond to cold calls.
